Request for Support at School of a Student's Health Condition
Some students may need to take regular medication/s or have a medication for a specific condition. If your child/ren need to take medication at school please discuss the details with the school Administration staff. Parents/carers will be required to complete a Request for Support at School of a Student's Health Condition, to be approved by the Principal, before medication can be brought to school.
Additional documentation, completed by a Doctor/Specialist may be required. Please contact the office to discuss your child/ren's health requirements.

Application for Extended Leave - Travel
Families are encouraged to travel during school holidays, however if travel in the school term is unavoidable and is for five days or longer, prior approval from the Principal will be required.
Families will need to complete an Application for Extended Leave - Travel Form and provide a copy of their travel itinerary (if travelling outside Australia).
